No one likes paperwork (including us), so we tried to keep it simple. Please complete both the registration form and wavier form for each hockey player. If you are registering two or more players, please fill out each form once per player.

Player Registration

Registration for the Fall 2024 season is open. Please complete the registration form and player waiver.

Player Wavier

A player waiver is required annually. If you are playing roller hockey with our program for the first time or it has been a year since you completed this waver, you must submit it for each player in the program.
